Circle of Care Education Award 2024

Application opens March 15

Due: April 15, 2024 (this includes all forms and letters being sent via email)

Applicant Criteria:

• Must be a past or current cancer patient
• Student pursuing a higher education degree/certification
• Accepted to or returning to college or technical school
• Age 17 - 23
• Treated at one of our partner hospitals:
  • Baystate Children’s Hospital
  • Connecticut Children’s Medical Center
  • Maria Fareri Children’s Hospital
  • Yale New Haven Children’s Hospital

Circle of Care provides emotional and financial assistance to families of young people with cancer with programs and services that meet the unique and challenging needs of childhood cancer treatment. This year, we're supporting our young adult community through financial assistance awards for their higher education. Each recipient will be invited to an award ceremony in June 2024 (official date and location TBA)

In addition to this form, you will need to submit...
• One letter of recommendation from an employer or school authority (teacher, guidance counselor, etc.)
• Most recent school transcript (can be unofficial) or proof of GED 
• Proof of acceptance to or attendance at one college or technical school
• Proof of diagnosis from your doctor or social worker
• One photo of yourself
